If you have been awarded a grant through our exisiting open funding schemes - New Works, Performance Groups, Promoters, Organisations or Festivals - using the online application system, you will now be able to submit your evaluation in the same way.
Simply click this link to access the online form, and once you have entered your email address and password, it will be ready to complete and submit.
For New Works and Festivals, fill in your application for after completion of the event.
For all other core schemes - Promoters, Performance Groups and Organisations - please fill in the form after the full year of funded activity has been completed (i.e. from the date stated on your offer letter).
If you applied for your specific funding prior to October 2009, you will need to complete the older evaluation form, downloadable as a PDF. See button on the left.
As part of funding award conditions, you MUST use the PRS for Music Foundation logo in any press and publicity material, including press releases, flyers, programmes, posters, emails and websites.

We now make all payments via BACS. If you have already received a BACS payment and your details are on our system, the remainder of your award will be paid to the same bank account as nominated for your first instalment.
If your details have changed since that point, please let us know so that we can amend our records. You can email the Applications Team with any questions regarding payments.
If you were previously paid by cheque, you will need to submit a Bank Details form with your evaluation to receive payment via BACS.
