If you have been awarded a grant through our exisiting open funding schemes using the online application system, you will now be able to submit your evaluation in the same way.
Please contact us when you are ready to complete your evaluation, using the subject line 'Evaluation Form Request'.
Only complete the evaluation form after all the funded activity has been completed.
If you applied for your specific funding prior to October 2009, you will need to complete the older evaluation form, downloadable as a PDF. See button on the left.
As part of funding award conditions, you MUST use the PRS for Music Foundation logo in any press and publicity material, including press releases, flyers, programmes, posters, emails and websites.
We now make all payments via BACS. If you have already received a BACS payment and your details are on our system, the remainder of your award will be paid to the same bank account as nominated for your first instalment.
If your details have changed since that point, please let us know so that we can amend our records. You can email the Applications Team with any questions regarding payments.
If you were previously paid by cheque, you will need to submit a Bank Details form with your evaluation to receive payment via BACS.
Please note that it may take up to three weeks to process payments.
