
You only need this form if you were a succesful recipient of a PRS for Music Foundation Award, and the project is now completed.
We are now making payments via BACS.
If you have already received a BACS payment and your details are on our system, the remainder of your award will be paid to the same bank account as nominated for your first instalment. If the details have changed since that point, please let us know so that we can amend our records. You can contact the Applications Team with any questions regarding payments by emailing applications@prsformusicfoundation.com
If you were previously paid by cheque, you will need to submit a Bank Details form with your evaluation.
New Evaluation Form
One of the most common requests you have made to us over the past few months has been the need for a new online evaluation form. We are here to support your creative work and make the process as smooth and efficient as possible. We hope you feel we listen and take action wherever we can to improve. If you have applied to our Core Grants (New Works, Performance Groups, Promoters, Organisations and Festivals) using our recently updated online application system, you will now be able to use the new online evaluation form.
